(1) Application Form of Overseas Supply Enterprise of Imported Cotton: 4 pages complete the form according to Filling Explanation and the original shall be offered.

(2) Effective business registration documents (copies): business registration documents refer to business registration of application enterprise (copies), enclosed with Chinese description, which states certificate type, content, license issuing authority, date of issue, etc.

(3) Basic materials of production/operation and quality control of supply enterprise: (to ensure that imported cotton meets the requirements of Chinese laws and regulations and trade contract, application enterprise adopts a series of measures or practices, which include quality inspection system, calibration system of measuring equipment, packaging control system of exported cotton).

Quality inspection system refers to the regulations or practices how application enterprise inspects self-yielding, purchased and marketed cotton.

Calibration system of measuring equipment: establish related rules of measuring instrument list, the quantity of equipments and regular or irregular calibration and maintenance of these equipments. The configuration of application enterprise shall meet the requirements of production, inspection and packaging. The materials may not be offered if supply enterprise does not have related equipments.

Packaging control system of exported cotton shall include whether the regulations abide by _ISO8115-1986(E)and adopt compressor suitable for ocean transportation to press export package and meet other requirements of packing materials (cotton cloth, PVC and other materials that do not produce foreign fibers).

(4) Institutional framework, department and job responsibility refer to department involved with quality control and management and the job responsibility, which can be described by text or block diagram.

(5) Data (plane graph) of stable premises or processing places refers to dimensional plane graph of fixed office address, processing place, dimensional place graph and picture information.

(6) Commitment to quality control of cotton (i.e. supply source and quality control of supplier): the contents shall be explicitly complete; original letter of commitment shall be offered with validity stamp or legal signature of supply enterprise.

(7)From trade activities of the previous year, there were no serious quality problems, harmful substances and bad record statement of serious consequence. Validity stamp or legal signature of supply enterprise shall be offered.

(8)Certification of application enterprise towards quality control method of supplier

Application enterprise shall offer related requirements and the acceptance method of supplier's quality control (submits a certificate of self-inspection or entrusted acceptance).
